PROBLEM TO BE SOLVED: To provide a form for placing concrete, which is not damaged when the form is removed after the concrete is hardened. SOLUTION: This plastic form 30 comprises a sheathing board part 32, one side of which serves as a concrete placing surface 31, and a side board part 33 which is perpendicularly bent from both side edges of the board part 32 to the opposite side of the surface 31 of the board part 32; and the board part 33 is provided with a longitudinal slot 40 for inserting a crowbar, whose opening is long in the longitudinal direction of the board part 33.
