PROBLEM TO BE SOLVED: To provide a questionnaire preparation system which makes it possible to acquire questionnaire replies with high effective reply rate.

SOLUTION: A questionnaire preparation server computer 23 groups monitors 11 based on rules according to monitor attribute data 19 and access log data 21 of the monitors 11, adapts the branch conditions of the questionnaire to the grouping rules of the monitors 11 and constructs a questionnaire scenario adapted for the group to provide a questionnaire form personalized for every monitor group.
