The surgical instrument has a pipe shaft (2) provided with a rotary effector (3) supported by an instrument head (4) at one end and an instrument grip (1) at the opposite end, with a pair of intermediate gearings converting the pivot movement and rotation of the instrument grip by the operator into a corresponding pivot movement and rotation of the effector. A movement compensation element is integrated in the intermediate gearing for the rotation movement for compensating the rotation of the effector upon pivot movement of the instrument head.
