ENTRY FORM, DEVICE FOR READING FORM INFORMATION, SYSTEM/PROGRAM F0R OUTPUTTING FORM INFORMATION

PROBLEM TO BE SOLVED: To provide a system for storing handwritten information to an entry form together with printing information and displaying these units of information according to necessity.

SOLUTION: In the system 500 for outputting form information equipped with the entry form 10 and a data entry pen 100, the entry form 10 has an IC chip 20 recording ciphered printing contents which can be visually inspected in the entry form 10 and coordinate specifying patterns 12 which is printed at an entry spot of the entry form 10 and specifies a coordinate on the entry form 10. The data entry pen 100 reads the printing contents from the IC chip 20, then recognizes the handwritten data to the entry form 10 based on the hysteresis of the coordinate specifying patterns 12 to be successively read while the handwritten data are entered and outputs both recognized handwritten data and read printing contents after making sure their corresponding state.
