摘要 <P>PROBLEM TO BE SOLVED: To provide a questionnaire information collecting system capable of accurately collecting questionnaire information from a customer for more efficient management and strategy of a shop. <P>SOLUTION: When an electronic mail address is transmitted from a cellular phone 1 of the customer A, a server 2 issues a bar code for the customer A to transmit it to the cellular phone 1 of the customer A. A bar-code scanner 31 at the shop reads the bar code from the cellular phone 1 of the customer A to transmit it to the server 2. The server 2 transmits a questionnaire asking mail to the cellular phone 1 matching the transmitted bar code for collecting questionnaire information from the cellular phone 1 of the customer A.
