PROBLEM TO BE SOLVED: To provide an inexpensive form support device for supporting a pair of opposed form rows stacked in upper-lower two stages, and securing long-term durability of a concrete foundation by restraining generation of rust from a concrete foundation surface. SOLUTION: This form support device 10 has a spacer 11 composed of a connecting bar 12 having a pair of installing holes 4 extending in the shaft direction on both ends of a straight steel bar-shaped body and a pair of nonmetallic connecting bodies 15 coaxially fixed to the shaft directional both end sides, having a shaft directional outside surface set at a right angle to the axis and provided with an inserting hole 16 for communicating with an end part of the connecting bar by extending in the shaft direction from the outside surface and a support member 21 having a base part 22 being a substantially rectangular plate and having a fixing hole 22a in a longitudinal directional one end side position and a bar-shaped installing part 23 integrally connected on the longitudinal directional other edge and extending in the extending direction of the base part.
