PROBLEM TO BE SOLVED: To raise a personal effect of a business effect or the like by describing more information of business and the like on a name card to make it function as a pamphlet for business and the like after submitting as the name card, and to provide the name card convenient for receipt and handling as both of a person using the name card and a person having received the name card. SOLUTION: A structure of a booklet wherein some sheets of oblong sheets 1 capable of being printed on both faces are bounded is taken, and after opening the page 1, adhesion is not again carried out. Each sheet 1 is bonded by an adhesion means to be carried out by an adhesion part 2. Binding of each sheet 1 is carried out by forming a rear part 3 wherein a rear side end part 1E of each sheet is bonded to constitute the name card 10 used also as the pamphlet.
