PROBLEM TO BE SOLVED: To enable, in a mold for forming a disc plate which is used when injection molding is applied to the disc plate, a stamper to be uniformly heated and cooled in a short time. SOLUTION: Concerning the mold for forming the disc plate wherein the disc plate is obtained by fitting the stamper 5 to at least one mold surface of the movable mold 1 and the fixed mold 2 which are oppositely disposed so as to be contactable/separable and by injecting and charging a melt resin material from injection equipment into the cavity formed in the state that the moving mold 1 is brought into contact with the fixed mold 2, the mold is comprised of the stamper 5 which is almost rectangular, has a center pore 5a formed at almost the center portion, and has a given transferring region 5b in the surroundings of the center pore 5a, and the electrode 17 which is disposed so as to come into contact with one outer edge 5c1 and the other outer edge 5c2 of the stamper 5.
