PROBLEM TO BE SOLVED: To provide a distribution and transportation slip, which can reconcile the more reduction of a price and the more upgrading (the more improvement of a service level) than ever even for a small-lot consignor, and its usage. SOLUTION: On concealing piece parts 15, 17 and 18, large letterings "cold, "storage" and "perishables" are respectively printed with a monochrome laser printer. On concealing piece parts 16 and 19, a separation-encouraging message such as "Please remove this portion." is printed so as to realize the sure selection of a desired distribution and transportation service. After that, when the concealing parts 16 and 19 are removed, from beneath the parts, the distribution and transportation service displaying part 26 of "cool refrigeration" and the distribution and transportation service displaying part 29 of "perishables" both printed in bright colors become visible.
