A unit that, in a surgical operation using an endoscope, for example, positions the front end of a surgical instrument in the region of a human body where a medical procedure is required, without spreading a hole formed in the human body for insertion of the surgical instrument. This unit comprises first and second output shafts pivotally connected to a movable member for the surgical instrument or the like, extending parallel to each other and reciprocable, and a drive means for giving the first output shaft an optional amount of advance/retraction and giving the second output shaft an amount of advance/retraction in a constant proportion (<> 1) at all times with respect to the amount of advance/retraction for the first output shaft.
