PROBLEM TO BE SOLVED: To provide a delivery slip which can use a release sheet as a duplicate slip. SOLUTION: The delivery slip 1 binds together a client duplicate 2, a delivery slip and receipt slip 3, a sales slip 4 and an attached slip 5 separably by a line paste 7. The slip 1 includes a copy structure so as to fill in the same item on the slips 3, 4 and 5 of lower sites by filling in the duplicate 2. An agency duplicate 6 having a release agent layer 20 is releasably adhered to a rear surface of the slip 5 via a pressure sensitive adhesive 21. The duplicate 6 prints an item necessary as the duplicate 6 at a previous stage of treating to release, and provides a self-color developing agent layer 19 which can be copy filled in.
