PROBLEM TO BE SOLVED: To provide an X-ray diagnostic apparatus for automatically performing positional adjustment of a top plate and/or an X-ray system so as to keep an X-ray exposure direction of the X-ray system with respect to the concerned site of a subject and a relative position of the X-ray system and the concerned site of the subject, or the relative position of the X-ray system and the concerned site of the subject, or a position of the concerned site of the subject about the direction of the body axis of the subject in the case of moving a keeping device for keeping the top plate for placing the subject and the X-ray system. SOLUTION: When the X-ray system performs the rotation movement of a rotation angleψabout the longitudinal direction of the top plate 1 to move an X-ray beam center axis from R<SB>1</SB>to R<SB>2</SB>, a control part decides the rising and falling angleθto beθ=ψ, and supposes the concerned site to have moved from Q<SB>1</SB>to Q<SB>2</SB>with the movement of the rising and falling angleθof the top plate 1 to decide a movement A' about the longitudinal direction of the top plate 1 and the movement B' about a vertical direction. Then, the positional adjustment of the top plate 1 is performed based on these values to keep the relative position between an X-ray exposure source and the concerned site Q of the subject.
