PROBLEM TO BE SOLVED: To obtain an image reader and an image forming apparatus which make output signals from a solid-state imaging device at respective image heights uniform in both a horizontal and a vertical scanning direction even when imaging positions at respective image heights in the horizontal and vertical scanning directions of an imaging lens are different, is adaptive to color documents, match imaging positions of respective colors R, G, and B with one another, make output signals from the solid-state imaging device uniform at the respective image heights, and cause neither deterioration in imaging performance nor shifts in imaging position in horizontal and vertical scanning directions of luminous flux on an optical axis. SOLUTION: The image forming apparatus has an imaging lens L1 which images image information of a document and the solid-state imaging device 400 which reads the image information. A mirror MA which has an anamorphic surface is arranged in the optical path between the imaging lens L1 and solid-state imaging device 400. COPYRIGHT: (C)2004,JPO
