PROBLEM TO BE SOLVED: To provide format configuration to improve the driving reliability of a high-density recording optical disk by a driver.

SOLUTION: An optical disk 100 is previously recorded with a manufacturer name and stamper serial number in a lead-in region 104 in the manufacturing state of a recording medium. The lead-in region further has management information which is the information relating to driving conditions of the driver of the optical disk, such as recording/reproducing/erasing conditions of the disk. Part of the management information is recorded as arithmetic management information 103 computed by using the manufacturer name and the stamper serial number. The driver decodes the arithmetic management information by using the manufacturer name and the stamper serial number and drives the optical disk when the normal management information can be acquired. The driving of the optical disk is suppressed even under the erroneous driving conditions by the driver by subjecting the management information to arithmetic processing by the manufacturer name and the stamper serial number and by recording the information.
