PROBLEM TO BE SOLVED: To realize recording and reproduction of multi-value information of constant linear density (CLV disk format) on the information track of an optical disk in a CAV drive state in which an ideal fundamental clock string (virtual CLV<SB>CLK</SB>) for forming the CLV disk format by CAV drive on an original disk medium or optical disk media is introduced and a comparatively stable drive state is easily realized. SOLUTION: A frequency in a CLV fundamental clock string for forming an information track is generated conforming to the following equation: (ω<SB>0</SB>×R<SB>0</SB>+P×ω<SB>0</SB><SP>2</SP>×n×T<SB>n</SB>/(2×π))/(LCB) wherein: n = 0, 1, 2, 3, etc.,ω<SB>0</SB>; rotation angular velocity of turn table, R<SB>0</SB>; exposure start radius position, P; track pitch, T<SB>n</SB>; arbitrary time interval of frequency updating, n; the number of times of updating formatter drive clock from start of exposure, L<SB>CB</SB>; fundamental length on formed CLV information track.
