<P>PROBLEM TO BE SOLVED: To easily set an information input form provided to a terminal having an information input function. <P>SOLUTION: The input of each information such as name information of set items, detailed information of the set items and the name information of input items is urged. When each urged information is inputted, the names of the set items and the detailed information are displayed correspondingly based on the information, and the information input form displaying information input areas corresponding to the names of the input items for every detailed information is prepared.
