<P>PROBLEM TO BE SOLVED: To provide a questionnaire method by which a user virtually experiences a service to be evaluated in a situation close to an actual service environment under different service providing conditions, and the characteristic of degree of satisfaction in a user is obtained through the use of the minimum kind of data, and also to provide a questionnaire device. <P>SOLUTION: The service in one grade among the plurality of grades concerning the prescribed service, and also the usage charge when the service of the presented grade is received are presented to a user terminal. The user is asked about his degree of satisfaction in the case that the user pays the presented usage charge and receives the presented service.
