<P>PROBLEM TO BE SOLVED: To solve the problem in a conventional document data auxiliary illustration picture plane by animation that a department for creating a document picture plane or animation picture plane is not necessarily one, and the content might be falsified in the distribution process of the produced picture plane. <P>SOLUTION: This electronic form creation device has member data for forming at least an electronic form, text data to be inputted, animation data to be inputted, an electronic form editing program for assembling each of the data into a style of electronic form, and an encryption key for electronically signing it. In this device, an electronic sign is given to both the electronic form created by the inputted text data, the member data and the electronic form editing program and the animation data inputted according to the text data by the encryption key.
