PROBLEM TO BE SOLVED: To provide a home care diary, in which the recording information on nursing services recorded on one booklet by a home help can be utilized on one hand as the diary of a home nursing care requiring person and, at the same time, on the other hand as the report to a home nursing service provider. SOLUTION: A large number of vertically coupled unit papers consisting of diary papers with each front page, on which the entry column of nursing services day, the displaying column of the contents of nursing services and the entry column of contacting matters are displayed in a predetermined layout, and report papers with each front page, on which the same matters as the displaying matters in the diary paper are displayed in the same layout as the predetermined layout, are piled up and bound and, at the same time, each report paper arranged on the side positioned lower in the unit paper is separably bound.
