PROBLEM TO BE SOLVED: To facilitate splitting of a concrete specimen relative to a concrete specimen formwork for forming the concrete specimen sampled from a part of a concrete structure, for example, for measuring the strength of the concrete structure. SOLUTION: Concerning a formwork for forming the concrete specimen, this concrete specimen formwork 1 provides the concrete specimen with a forming slit plate for smoothing a cut-out face of the concrete specimen, having a smaller opening part 2a than an opening part which is the opening part for introducing and filling into the formwork the concrete placed in the concrete structure.
