<P>PROBLEM TO BE SOLVED: To create a questionnaire with high questionnaire creation efficiency and in conformance to both types of online and printed questionnaires. <P>SOLUTION: Along a questionnaire creation procedure, a prepared template is used to create questions and choices of a questionnaire. For on-screen answering to the questionnaire, branch screens from choices are set to present screens of the next questions depending on selection from the choices.
