PROBLEM TO BE SOLVED: To enter same information such as information on an altered item in a plurality of communication media to different addresses by entry at one time and to simply send the same notification to the different addresses in the form of a sealed letter. SOLUTION: A first notification sheet 2 having address information 9 and a second notification sheet 3 having different address information 9 from that of the first sheet 2 are stuck together separably. The information entered in an information entry space 8 of the first sheet 2 is copied simultaneously on the information entry space 8 of the second sheet 3. The sealed letters 10a and 10b being different in the address information 9 and having a mailable formare obtained respectively from the first sheet 2 and the second sheet 3 are made to have the same information by the simultaneous copying.
