PROBLEM TO BE SOLVED: To transmit form information under uniform rule conditions irrelevantly to form conveyance constitution. SOLUTION: Sensors A, B, C and D are arranged on a form conveyance path 77 from upstream to downstream, a pickup roller 73, a registration roller 72, a conveyor roller 73, and a registration roller 75, and a conveyor roller 73 which send paper out to respective sensors are arranged, and an FIFO stack type 1st form information memory which stores form information by sheets of paper by sections corresponding to the respective sensors and a 2nd form information memory which stores form information by the sections are included; as forms are fed to the respective sensors, form information is moved from the upstream or downstream 2nd form information memory to the 1st form information memory and as forms are put on edges of the respective sensors, form information is moved from the 1st form information memory to the 2nd form information memory to transmit the form information from upstream to downstream, thereby performing form conveyance control by using the form information.
