PROBLEM TO BE SOLVED: To provide an optical recording medium which permits highly accurate and stable recording and reproduction improved in surface format efficiency and also adaptable to random access. SOLUTION: The optical disk 10 has two recording layers, and information can be recorded/reproduced to/from spiral tracks 12, 13 of a selected recording layer by emitting light to one side face. One or a plurality of index header fields 14 are formed in the recording layers so as to intercept each track, and in the index header field(s), address information formed of embossed pits is arranged in a constant circumferential direction and at a position shifted by a predetermined quantity in the adjacent track or every two or more tracks.
