PROBLEM TO BE SOLVED: To relax the impact of a lid body, which opens and closes the opening part of a concrete form, and the concrete form. SOLUTION: The lid body 4, which opens and closes the opening part 1A of the concrete form 1, is pivotally supported in a freely revolvable manner. The lid body 4 and the support frame 5 of the concrete form 1 are connected by a second damper 12 and the lid body 4 is opened and closed at the time of filling with concrete and at the time of demolding. When the lid body 4 is opened, it is supported by the second damper 12 so as to be made openable and closable by light force to relax the impact of the lid body 4 and the form 1.
