PROBLEM TO BE SOLVED: To provide a label slip which is adequately usable in medical instutitions, manufacturers of medicaments and medical implements, sale makers, etc. SOLUTION: The label slip 1 is formed by temporarily bonding a carrier piece 5 onto a mount 2, temporarily bonding a label piece 8 onto the carrier piece 5, dividing the label piece 8 by a first disconnecting line 9 formed along the transverse direction thereof to a first printing surface 10 printed with visual information x and second printing surface 11 printed with bar codes x, and further forming a second disconnecting line 14 approximately parallel with the first disconnecting line 9 to the carrier piece 5 by slightly offsetting the line to the printing surface 10 side to form a first divided surface 15a carrying the printing surface 10 and a second divided surface 15b carrying the printing surface 11.
