PROBLEM TO BE SOLVED: To provide a hydraulic shock absorber having a rod guide structure preventing operation oil inside a cylinder from leaking outside through a slide contact portion of a main seal and a piston rod, even if a rod guide and the main seal are arranged adjacent to each other. SOLUTION: An outer shell 2 and the cylinder 1 are concentrically arranged, and the rod guide 3 is provided extending from an end of the outer shell to an end of the cylinder. The rod guide 3 has a guiding part to guide a piston rod to its center, and a main seal provided on an upper part of the rod guide. A reservoir chamber partitioned between the cylinder and the outer shell is connected to an oil chamber in the cylinder through a drain hole formed in the rod guide, an oil passage provided between the rod guide and the main seal, and a sliding clearance between the guiding part and the piston rod 10. In the middle of the oil passage, a check seal 13 is provided to prevent gas from the reservoir chamber R from entering to the oil chamber in the cylinder. The sliding clearance and the oil passage are connected to a tip of the guiding part and an operation oil exhaust passage 22 greater in open area than the sliding clearance for jet speed dispersion is provided.
