A three-dimensional (3D) model is created (200) in response to cardiac image data created from acquisition data obtained from a medical imaging system. Three anatomical landmarks are identified on the 3D model. The 3D model is sent to an interventional system in a format that can be imported and registered with the interventional system. Independent claims are also included for the following: (1) system for creating three-dimensional model for cardiac interventional procedure planning; and (2) program for creating three-dimensional models for cardiac interventional procedure planning.
