An opioid analgesic dosage form to reduce the abuse potential of the active opioid analgesic, comprising an analgesically effective amount of an orally active opioid agonist combined with an opioid antagonist so that at least a two-step extraction process would be required to separate the combination, the amount of opioid antagonist including being sufficient to counteract opioid effects if extracted together with the opioid agonist and administered parenterally.
