PROBLEM TO BE SOLVED: To provide an irradiation field forming apparatus which increases the efficiency of irradiation of a charged particle beam to an affected part, and does not have a fear of deteriorating a device inside the apparatus. SOLUTION: A wobbler electromagnet 1 for deflection in the direction X; a wobbler electromagnet 3 for deflection in the direction Y arranged on the downstream side of the wobbler electromagnet 1; and a scattering substance 4 which is arranged on the downstream side of the wobbler electromagnet 3, has a thick portion 4a on the center side, an had a thin portion 4b on the external peripheral side of the thick portion 4a; are provided.
