PROBLEM TO BE SOLVED: To provide a mail form with which a part of mail prepared by using the mail form can be certainly cut from previously formed perforations for cutting, even when the mail is to be partially cut. SOLUTION: This mail form is constituted of a sheet comprising a plurality of unit paper pieces connected together through a plurality of perforations and a re-releasable dummy adhesive layer formed on a face scheduled for adhesion. The perforations for cutting formed in the crosswise direction for cutting part of the unit paper pieces when the mail form is folded at perforations for folding and perforations for folding/cutting, are formed in the unit paper pieces positioned outside the perforations for folding and cutting. In addition, the perforations for cutting are formed of a linear perforation line extending crosswise which consists of a plurality of cut parts and uncut parts and obliquely cut parts which commence from one end of each cut part of the cut parts and oblique in the obliquely downward direction to the linear direction of the linear perforation line.
