PROBLEM TO BE SOLVED: To reduce any disagreement between a commodity to be delivered, a delivery slip, and a packaging. SOLUTION: A sheet for the delivery slip having a column of information required for the commodity delivery is provided on a packaging sheet or a packing member in a peelable manner. The sheet for the delivery slip has a column for receipt signature or receipt stamp of a delivery destination to prove presence/absence of at least the fact of delivery, and the so-called role of a slip for proving the delivery is played.
