PROBLEM TO BE SOLVED: To accurately and easily connect an optical waveguide and an optical fiber of an optical switch. SOLUTION: The optical switch 101 is provided with a plate main member 1 containing, a plurality of optical waveguides 11a which linearly extend so that the optical waveguides cross at a plurality of crossing points, and is provided with optical fibers 3. The main member 1 and the optical fibers 3 are connected via a connecting member 2. The main member 1 has slots 13 which are so provided to cross the optical waveguide 11a. The connecting member 2 contains a plurality of optical waveguides 11b which linearly extend for the transfer of an optical signal between the optical waveguide 11a and the optical fiber 3. The main member 1 and the connecting member 2 are branched from a continuous member on a same substrate, and the optical waveguide 11a and the optical waveguide 11b are branched from a continuous optical waveguide on a same substrate.
