<P>PROBLEM TO BE SOLVED: To provide an identity verification system by an image at a medical site capable of preventing medical errors such as patient mix-up at an operation and in medication by accurately confirming the name and face of the patient and of protecting the privacy of the patient. <P>SOLUTION: Patient data D constructed by including face data f obtained by making the face of the patient P data and personal information are memorized in a memory device 1, written in the memory device 1 by a reader/writer 2 and can be sequentially updated. Meanwhile, a data carrier 3 with identification information d for referring to the patient data D of the patient P memorized is attached to a nameplate 4 written with a name N, and the face data f memorized in the memory device 1 is displayed as an image picture on a displaying means 5 on the basis of the identification information d read out of the data carrier 3 by the reader/writer 2.
