<P>PROBLEM TO BE SOLVED: To input medical treatment data on an electronic chart like handwriting on a doctor-friendly sheet of paper for the chart. <P>SOLUTION: Data are obtained from a formatted picture on the sheet of paper for the chart by means of optical reading or the like. Data display is applied to the formatted picture to accept input operation for the medical treatment data.
