PROBLEM TO BE SOLVED: To provide a device and method for test disk formation, which can easily form an adequate test disk.

SOLUTION: The method comprises: dividing a data area of a writable disk into a plurality of recording areas and setting conditions of recording by a recording means by the plurality of recording areas; adapting the recording means to the recording conditions by the recording areas and recording specified information data to the data area of the disk by the recording means; adapting the recording means to specified optimum recording conditions and recording data for readout processing to a readout area of the disk by the recording means after the recording means records the specified data to the data area of the disk; and further recording data for finalization processing to an information management area including a read-in area of the disk by the recording means.
