PROBLEM TO BE SOLVED: To raise a recovery rate of filled-in questionnaires from several to several tens of times as much as a conventional one even when a special merit such as a user registration, a present provision or the like is not imparted to users. SOLUTION: The magazine with the questionnaires comprises an envelope 1 in which an address 31 of a predetermined return destination is printed, a magazine body 3, an application sheet 2 of a plurality of application sheets for the application to provide the present introduced in the body 3 and/or a prize and different addresses are printed and to be filled in the envelope 1, and a questionnaire sheet 4 of the sheet 4 having a questionnaire fill-in part 4a for filling in information regarding oneself by an applicant using the sheet 2, filled together with the sheet 2 in the envelope 1 and sent in such a manner that the envelope 1, the sheet 2 and the questionnaire sheet 4 are integrated with the body 3.
