PROBLEM TO BE SOLVED: To improve operability, safety and operational efficiency and to provide a high- accurate medical operation, or the like by decelerating the displacing speed of the non-working surface of a scalpel part rather than the speed of the working surface thereof by outputting longitudinally twisted synthetic vibrations in the scalpel part on the distal end of an ultrasonic horn by applying converting processing to longitudinal vibrations transmitted from an ultrasonic oscillation mechanism. SOLUTION: This compound vibration ultrasonic hand piece has the ultrasonic oscillation mechanism composed of a longitudinal vibrator, a lining board and a front panel to output ultrasonic vibrations, the horn connected to the ultrasonic oscillation mechanism to amplify the transmitted vibrations, a vibration converting mechanism for converting the vibrations transmitted from the ultrasonic oscillation mechanism to synthetic vibrations composed of the longitudinal vibrations in the direction of the central axis of the horn and twisted vibrations with the central axis of the horn as a supporting point, and the scalpel part having the working surface and provided on the distal end of the horn. The vibration converting mechanism is composed of one or more groove parts formed on the peripheral surface of the horn or of the lining board, and the scalpel part is formed with a change gear mechanism for the twisted vibrations in the synthetic vibrations to decelerate the reciprocating rotation speed of the non-working surface rather than the speed of the working surface.
