FIELD: medicine, reconstructive microsurgery. SUBSTANCE: at rib below serratus muscle one should dissect periosteum along inferior costal edge laterally against fixation site of external intercostal muscles frontwards and backwards along external costal surface being in accordance to desired sizes of fragment's osseous part by length, along the line of periosteal dissection one should carry out osteotomy to the depth up to trabecular bone, moreover, osteotomy lines along external surface should be ended at the site of necessary fracture line along the edge of transplant's osseous part formed in the upper costal part, then transplant's osseous part should be broken, from another rib below serratus muscle one should dissect periosteum from external costal surface in the space between external intercostal muscles to take a fragment that saves perforating vessels and provides osteogenesis.
