PROBLEM TO BE SOLVED: To solve a problem which press molding is poor in a versatility of a conventional die processing by a micro grinder which only molds such as a V-groove for an optical-fiber guide, although parts of the same shape are manufactured in large quantities and inexpensively.

SOLUTION: A die is obtained by using a micro electron discharge method. It has been difficult for the die to be produced by grinding by a micro grinder. An optical mounted substrate is manufactured by press forming using the die. An optical fiber, an optical waveguide, a lens, an isolator, an optical filter, a light receiving element, and a light emitting element are mounted in a passive alignment. A multi-functional optical device and module are manufactured inexpensively in large quantity.
