PROBLEM TO BE SOLVED: To provide an OCR form, a form issuing system and a form reading system that can reduce the load of a reading work and enhance the working efficiency. SOLUTION: The OCR form 10 for reading entries by optical character recognition stores layouts 121, 122, 123, 124, 125 serving as a guide when the entries are filled and printed with drop-out colors, and a two-dimensional bar code 13 which stores lay-out data corresponding to the layouts 121, 122, 123, 124, 125 and is printed with drop-out colors. |
