The sleeve (10) has a lower carrier layer (11) and a cover layer (12). The carrier layer and the cover layer are connected together at their edges leaving an insert opening (21) for the disk. At the opening the cover layer extends beyond the carrier layer forming a tab (13). The carrier layer and the cover layer are formed of a plastic film and the cover is provided with a line of perforations (19) near the opening. A tear out strip may also be provided in the cover layer.
