PROBLEM TO BE SOLVED: To easily and appropriately construct a base foundation part of a foundation using base foundation forms simple in structure and easily arranged. SOLUTION: This foundation form structure 10 used to construct the base foundation part 1 of the foundation is provided with a lateral pair of base foundation forms 13, 13 for forming the base foundation part, and base separator plates 11 disposed at the bottom part of an excavated part and having a lateral pair of locking parts 12, 12 for respectively locking the lower end parts 13b, 13b of the base foundation form, and hole parts 12a, 12a formed on the outer end side of the respective locking parts. The base foundation form is a steel plate member forming the side wall of the base foundation part and provided at its upper end with an outward bent horizontal edge part 13a which is provided with a plurality of reinforcing bars 14 supported in a vertically movable manner along the plane direction of the plate and driven into the bottom part of the excavated part through the hole parts of the base separator plates when going down.
