PROBLEM TO BE SOLVED: To provide a device and a program for inputting an electronic form added with convenience by enabling customizing the electronic form for every user. SOLUTION: Input interface specifications including image specifications of the electronic form for the user are previously set in a using environment information setting section 23. When using the electronic form, the electronic form is customized for every user by reading out the input interface specifications including the image specifications previously set by an electronic form generating section 26 and reflecting them in the electronic form to be used.
