PROBLEM TO BE SOLVED: To provide optical information output device and reader capable of attaching a large amount of information by being capable of obtaining wide chromatic dispersion and also improving an SN ratio of the information by simultaneously accomplishing high diffraction efficiency. SOLUTION: This optical information output device disperses one laser beam including a prescribed wavelength area in a diffraction angle direction corresponding to a wavelength by making the laser beam pass through a first diffraction element, deflects the dispersed light to parallel light, attaches information to light of each wavelength passing through an information attaching element by making the deflected light pass through the information attaching element, subsequently converges the light of each wavelength, and outputs the converged light as one laser beam again by making the converged light pass through a second diffraction element. The optical information output device and reader uses a deep-groove type diffraction element as the first and second diffraction elements.
