PROBLEM TO BE SOLVED: To provide a form for preparing a simplified grating crib, which is capable of measuring its mortar covering thickness and in addition, being folded up, easy to be stored and transported, and further excellent in economic efficiency and followability to the ground surface where it is installed; and also provide its preparation method. SOLUTION: A form for preparing a simplified slope form having at least a pair of side nets 10 facing each other, and space retainers 20 each running laterally between the pair of side nets 10, and a hardener is sprayed to prepare a grating crib into a semi-cylindrical shape in section, which is characterized in that an inspection/measuring gauge 21 that can measure cover layer thickness on the grating crib is formed on the spacer 20, with its one part being fixed to the spacer 20 and its other part being movably inserted into the side nets 10, and they can be folded up; and also a method to prepare such a simplified grating crib by means of the form 1.
