PROBLEM TO BE SOLVED: To provide a delivery slip which is properly stuck to an article to be delivered in the process of delivery and has at least a part, with an entry of delivery information, to be removed from the slip stuck to the article, so that the delivery information can be easily used even after the article is delivered. SOLUTION: A specified number of slip copies 7 are arranged, in a freely separable manner, inside an envelope part 6 having a delivery information entry part, with three peripheral sides where an upper paper piece 3 and a lower paper piece 4 are lapped over each other. In this delivery slip 1 of the described structure, an attaching agent is provided on the back of the lower paper piece 4 and a release paper 8 is also provided, in a freely releasable manner, which covers the attaching agent. Further, a cutting part 9 is formed inside a sealing part 5 covering the three sides of the upper paper piece 3.
