PROBLEM TO BE SOLVED: To provide a pocket size booklet for holding an adult's record so as to hold a statistical record from a specific person's birth. SOLUTION: A health and immunity recording booklet comprises water resistant pages which are each made of a synthetic material and are not broken in such a manner that data are printed or written on the pages with an unerasable ink. Each page has a watermark. Each page can record a personal or child medical history, a body state, an operation, a drug and a vaccination. The booklet also comprises a page in which a neonate's footprint and fingerprints are disposed.
