PROBLEM TO BE SOLVED: To quickly and accurately request a test from a medical institution to a clinical test company by using an electronic form reflecting the information of latest test items. SOLUTION: A person in charge in the medical institution 9 refers to test item data 28 from a computer 33, inputs the test to be requested to the electronic form 26 of a test request and transmits it to a WWW server 21. The WWW server 21 converts a medical business division code unique to a hospital inputted to the electronic form 26 to a code for a test company, inputs it to the electronic form 26 and transmits the electronic form 26 to a test consignment reception computer 15. Also, the medical institution 9 outputs a barcode 39 on the basis of data inputted to the electronic form 26 and sticks it to the container or the like of a specimen 40 to be submitted to the clinical test company 5. In the clinical test company 5, the specimen 40 is tested according to the electronic form 26 of the test request.
