PROBLEM TO BE SOLVED: To provide a card form with a non-contact IC card which can reduce the cost, increase a recording information amount, and facilitate data processing. SOLUTION: The card form 1 with the non-contact IC chip is provided with a specific print field and holds a card 3 with a contactless IC chip in a peelable state; and the card with the non-contact IC chip is formed of a three-layered laminated body of two temporarily bonded two layers of thin-layer plastic films and a card form base material 1b connected to the form and the card form 1 can be separated from the form by peeling off the card 3 from the temporarily bonded layer 6 along a half-cut line 4 punched extending from the card form base material to the temporarily bonded layer 6. On the card paper base material of the card with the non-contact IC, an antenna pattern is printed in transparent conductive ink on the side of the thin-layer plastic films and a seal with an IC chip is mounted on the antenna pattern. |
